Students' Disciplinary Records related to violations of the Alcohol Policy and the Instrument of Student Judicial Governance are maintained in Student Conduct. Disciplinary Records include, but are not limited to, records related to Alcohol Policy violations and Honor Code violations.

If you have a document that must be completed by Student Conduct or need a copy of your disciplinary history records sent to someone other than yourself, please complete the Disciplinary History Request form with the following information:

• Email address or link that will be used for forwarding the completed document to the appropriate individual or entity
• Documents that have been completed by all other appropriate offices or University officials, if applicable

Please note that it may take up to ten (10) business days to complete and process your request. Information provided by the Student Conduct is separate from information related to your academic records provided by the Office of the University Registrar. If you are requesting information regarding your academic record, send your document to the Registrar.

FERPA WAIVER

In accordance with the Family Educational Rights and Privacy Act (FERPA), the University is prohibited from releasing certain information contained within student educational records, including disciplinary files maintained through Student Conduct. This restriction applies to, but is not limited to, parents/guardians, partner/spouse, and sponsor.

At the discretion of the student, they may grant permission to review information within their student records with a third party by completing the form below and waiving privacy rights.

Complete this form to allow a third party to be present during respective conduct meetings*, as requested by the student, and/or review student electronic records.

After completing this form, you may request to review your electronic records with a third party by submitting a completed electronic records review form and scheduling a meeting with Student Conduct by emailing jpa@unc.edu. The respective parties will be able to view the file virtually over Zoom, or schedule a time to view a paper file in the Student Conduct Office. The student must be present with third party to view the file.

*In accordance with the Instrument, exceptions may apply to third parties permitted to attend conduct meetings.
